REPORT A CLAIM

Please report all claims and notices of injury/incident to LCI Workers' Comp in one of three ways:

• Download the First Report of Injury Form, fill it out, and fax it to (985) 612-6741 or email it as an attachment to claims@lciwc.com
• Call LCI Workers' Comp at (888) 246-1988 during normal business hours, Monday through Friday, 8:00 AM to 5:00 PM.
• Email information regarding the injury, including the name of your company, the name of the injured, the date of the injury, and a description of the injury to claims@lciwc.com and someone will call you to complete the First Report of Injury Form.

Note: After you fax or email the First Report of Injury Form to LCI Workers' Comp, please always call LCI at (985) 612-6741 during normal business hours to confirm that your fax or email has been received.

In the event of a medical emergency please always first attend to the medical needs of your employee. If you need to report an emergency after normal business hours to LCI please call (504) 258-3226.
